The most important element is that you're continually monitoring their condition. That means they need to be awake to answer questions. Sleepiness and lack of shivering are BAD signs.

Also: plan. If you're going to be in that type of area, know your water sources. Ask locals. Take a contour map and mark the area. Tell people where you're going, leave a trip plan with the local ranger station, be prepared.
